Below are instructions for completion of forms that must be completed and delivered to the Bank to report forged or altered items.

AFFIDAVIT OF FORGERY OR ALTERATION

Complete the Affidavit of Alteration in cases of forged or altered monetary instruments (i.e.: personal check, cashier’s check.)

On this form, your signature must be notarized; for your convenience, please bring the unsigned affidavit of alteration to your nearest branch location for an on-site notary service.

(To better ensure your visit will be prompt, it’s advisable to call ahead to verify a notary is available at the branch you plan to submit the form to.)
